Risk Identification and Projection of Outcomes
TrestleTree has the ability to perform stratification on an employee population based on medical claims, prescription claims, biometric data and either internal or third party health risk questionnaire data. TrestleTree will utilize a proprietary "risk interrogation system" to identify:- The "at risk" population to be targeted by TrestleTree
- The current economic burden associated with this population, emerging risk and existing disease states
- Total costs associated with lifestyle modifiable risk factors
- Total costs associated with chronic illness

Disease Management
Participants work with their own Health Coach to better understand their condition, manage it and maintain control over it. The model uses a whole-person coaching philosophy that addresses each individual’s unique blend of conditions and disease states as a whole rather than focusing only on the individual's eligible conditions. Participants manage their disease state(s) while simultaneously changing the underlying behaviors that contribute to the development and progression of those disease states.
- Helps individuals to achieve tangible health behavior change toward physician directives and nationally-recognized evidence-based medicine guidelines/best practice guidelines
- Goal areas including nutrition, exercise, medication adherence, disease state monitoring and stress management
- Co-morbidities such as tobacco cessation and weight management, in addition to co-morbid conditions (e.g., back pain, cancer), are also incorporated into the coaching as applicable
- Understand their health and their life in depth,and develop a comprehensive plan that works specifically for them
- Disease-specific barriers to change are addressed through the course of coaching as an individually-specified plan for change is developed
- Ensure appropriate disease-specific screenings and tests as recommended
- Laboratory measures are collected during each appointment via self-report or as made available through objective methods such as biometric screenings

Prevention and Wellness
TrestleTree’s Prevention and Wellness Program is a comprehensive coaching program that is multi-behavioral focused and includes tobacco cessation, obesity and weight management. Co-morbidities such as disease states and associated behaviors such as medication adherence are also incorporated into the program. Participants work with their Health Coach to identify goal areas and the coaching supports behavior change progress toward participant goals, physician directives and evidence-based medicine guidelines.
- Incorporate learning from previous quit attempts
- Understand the unique motivations and barriers of each individual
- Aim to increase social support and coping strategies for triggers and cravings
- Include relapse prevention techniques
- Rigorous clinical review process of referrals for NRT or tobacco cessation helper medications by an internal pharmacy team
- Ability to serve as a Prior Authorization agent
- "Whole health" approach (e.g., increase exercise, change poor food habits, handle stress)

Assess

Nutrition
- Improve food intake in a way that meets the Dietary Guidelines for Americans and the Food Guide Pyramid
- Participants work to make attainable and sustainable changes
- Takes into account participants’ likes and dislikes cultural aspects, family concerns, etc.
- Help improve the nutrition of the family
Exercise
- Either sustain and enhance a current exercise routine or begin a new exercise program
- Find ways to include exercise in routine that meets the guidelines established by the American College of Sports Medicine
- Includes aerobic, strength, and flexibility exercises
- Participants work toward meeting their goals in the context of their lives, families, and health status
Stress Management
- Highly individualized approach where the unique life circumstances and sources of stress for each participant drive the content of coaching
- Identify triggers and stress symptoms
- Monitor symptoms via stress diaries or logs
- Develop a repertoire of stress management techniques (e.g., relaxation techniques, deep breathing and muscle relaxation, cognitive challenging, time management, utilizing social support)
- Provide referrals for resources available to participants (e.g., EAPs, community resources)
- Increased efficacy for coping with stress and a sustained reduction in symptoms.
